Antibiograms: “What and Why?” #CMDA2018 Annual Conference

Antibiograms: “What and Why?” “A collection of data usually in the form of a table summarizing the percent of individual bacterial pathogens susceptible to different antimicrobial agents” Because susceptibility can vary even within a species (with some strains being more resistant than others), antibiotic susceptibility testing (AST) is usually carried out to determine which antibiotic will be most successful in treating a bacterial infection in vivo. https://www.merriam-webster.com/medical/antibiogram

Antibiograms: “What and Why?” Antibiograms are often used by clinicians to assess local susceptibility rates Antibiograms are used as an aid in selecting empiric antibiotic therapy Antibiograms are used in monitoring resistance trends over time within an institution Indian J Med Microbiol 2010 28(4):277-80

Antibiograms: “What and Why? Klebsiella resistance to Merepenem 2000 to 2012

Antibiograms: “What and Why? Acute Change in Condition McGeer’s criteria or Loeb criteria met for likely infection Start Rx before bacterial culture results back (to avoid development of Hospitalization/SIRS/Sepsis or DEATH Know likely organisms for this site and the likely resistance patterns for this facility

Antibiograms: “What and Why?” Beyond scope of this talk

Antibiograms: “What and Why? Who do you include: Your own facility: but low # isolates City-Wide LTC Antibiogram (ask your lab) Referring Hospitals: where you new bacteria come from: UCH several years ago had outbreak of resistant bacteria (MDRO)

Antibiograms: “What and Why?

Antibiograms: “What and Why? How often are they updated: Most commonly annually at Hospitals Antibiotic Stewardship Program should track when new antibiograms are due (lab, hospital, etc) Communicate to providers when new resistance trends are noted
