Patient Safety and Quality Improvement Act (PSQIA) The PSQIA is a federal law, effective July, 2005 with final regulations published November, 2008 Provides for establishment of Patient Safety Organizations (PSOs), which are private or public entities that are federally listed by the Secretary of the US Department of Health and Human Services (HHS). Purpose of PSOs: Provide a “safe haven” for health care providers to improve patient safety and quality of health care Provide an avenue for voluntary and confidential reporting of near misses, events and unsafe conditions, which can be used for learning Center for Patient Safety Toolkit for PSO Participation, Section 4

Center for Patient Safety Toolkit for PSO Participation, Section 4 Our PSO Participation (Name of Organization) is a contracted participant in the (Name of PSO) (Name of PSO) (location of PSO, date certified as a PSO and PSO services provided) (Name of Organization) participation in the (Name of PSO) provides legal protection and processes for reporting, learning and sharing to improve the safety and quality of care provided Center for Patient Safety Toolkit for PSO Participation, Section 4

Why We Participate in a PSO Share information with the (Name of PSO) so others across the state can learn from each other Take advantage of the legal protection and confidentiality which allows us to learn from our near misses, events and unsafe conditions Center for Patient Safety Toolkit for PSO Participation, Section 4

Patient Safety Work Product (PSWP) (Name of organization) defines the following as PSWP: (Enter your definition of PSWP) Occasionally you may be aware of information considered PSWP.
