Importance of the Form 48 Serves as a projected plan for: Field Training Graduation Commissioning Determines if goals are realistic Creating the best plan for your success

Administrative Data Academic Plan Filling out a Form 48 Block 1-6 (Blue) Initial page only Academic Plan Term & Course information (Red) Two primary components: Administrative Data only completed once on initial page of the Form 48. This section contains the general information such as name and academic major that (should but is not restricted to) remain constant throughout the cadet’s AFROTC career. Academic Plan is recurring section for remainder of document. This section contains the information for every academic term for the cadet’s proposed academic plan.

Block 2 – Academic Institution (Pen) Block 3 – Academic Major (Pen) Administrative Data Block 1 – Name (Pen) Block 2 – Academic Institution (Pen) Block 3 – Academic Major (Pen) Administrative Data has 6 section/blocks each requiring a single piece of information. Block 1 is simply meant for the cadet’s full name in the order as requested and should be written in pen. Block 2 is meant for the cadet’s currently attended university name and detachment (in this case the detachment is always Det040). Block 3 is meant for the cadet’s current or prospective academic major. This needs to be verified by the cadet’s university catalog to avoid any spelling errors or synonymous majors such as Aerospace Engineering Vs. Aeronautical Engineering. Each of these sections are to be written in pen.

Administrative Data (cont.) Block 4 – Institutional Official Review (Pen) Block 5 – Initial Review (Pencil) Block 6 – Final Signature (Pen) The other 3 sections from Administrative Data are equally important. Block 4 is used after a cadet has completed an advising session with their academic advisor (from their university) to verify their proposed academic plan is realistic and possible. The advisor will sign and date in the block in pen. In the event the advisor refuses, the cadet’s APAS is allowed to replace the advisor’s signature with their own during Term Counseling. Block 5 is meant to be completed in pen but initial should be done in pencil. Depending on the cadet’s academic major, their intended number of years may be advised to increase or decrease whether from their academic or AFROTC advisor. This block will have the cadet’s degree type (BS or BA) and the anticipated year of graduation. Block 6 is meant for a final review by the cadet ONLY after graduating. This block will not be “touched” in any shape or form until the cadet has reached this point.

The four main components

Verify Full-time status Form 48 Reminders Verify Full-time status Specified by institutional catalog 12 unit hours undergraduate 9 unit hours graduate Confirm 2 academic year minimum for Professional Officer Corps (POC) Leave room for winter and summer terms Leave a “Summer ____” term blank for Field Training Full-time student status is an absolute requirement to continue participation in training for all cadets. Although the “standard” number of units specified for achieving this status is stated in the AFROTCI 36-2011, the actual number is situation based, meaning it may vary depending on a cadet’s university course catalog. Another huge requirement is the cadet having at least 2 academic years (4 semesters or 6 quarter terms) set for training as a POC. Cadets are allowed more time as a POC but 2 academic years is an absolute minimum. As a method to simplify adjustments, it is highly advised to create an electronic version of the Form 48 using a spreadsheet program such as Microsoft Excel. This allows changing courses and full time student status verification to be done quickly without the incessant need to erase and write repeatedly.
