Clinical Placement Program in Cancer Survivorship 2017-2019 Prepared for VPHNA – August 2018

Clinical placement program Context Increasing numbers of survivors 5-year Victorian cancer survival rates 46% (1986)- 67% (2011) Increasing role of Primary Care during survivorship phase With growing requests for education and development Previous successful pilots at Peter Mac and partner hospitals (2015, 2016) Participants reported program was clinically and professionally valuable 2017-2019 - program objectives Improve knowledge and confidence for primary care professionals (PCPs) caring for survivors Improve understanding of the discipline-specific roles required for survivorship care Enhance links between the community and hospital based professions

Clinical Placement Program in Cancer Survivorship 2017- 2019 Victorian State Government funding provided to ACSC to deliver a state-wide program: Minimum 8 tertiary host sites Placements offered from Jan 2018- April 2019 Target > 100 PCPs. Current recruitment (Aug 2018): GPs = 43 (target >70) Nurses = 18 Allied Health = 26 Thanks to the success of the last two placements- funding received from DHHS to provide further placements with a view to embed placements in survivorship across 8 hospitals and will run from Jan 2018– April 2019. Minimum of 100 PCPs will attend a 7-10 hour placement in cancer survivorship. Promotion through hospital networks, PHNs and organisations including RACGP/ACRRM/APNA .. Positively received with more than 40 applications since Nov 17. Tailored workshops created in collaboration with existing survivorship programs to address the learning needs of PCPs in addition to the placement.

The Placement Observational placement Attendance at 1x MDT meeting plus 2-3 multidisciplinary clinics/survivorship GPs attend 2-3 specialist clinics (7-10 hrs total) Primary Care Nurses & Allied Health attend 2x specialist clinics (Maximum 7 hrs) Applicants must demonstrate an interest in cancer care Application, Personal Learning Goals, Post-Program Survey Observe and discuss the decision making process around treatment planning for patients with early stage and advanced cancers in a number of different cancer types. There will also be a focus on post-treatment surveillance and monitoring
