Considerations Must be accurate Always obtained on admission to a Health Care Facility
Guidelines Obtain the same time each day Make sure scale is balanced at “0” before weighing pt Use the same scale each time Try to make sure pt is wearing the same amount of clothing Remove extra clothing, as jackets, sweaters
Safety Always keep safety in mind * Do not leave pt alone *Assist on & off of scale *If pt not wearing shoes, place paper towel on scale
How to record Both Metric and U.S. standards are used Ht is measured in centimeters (cm) or inches (in) 1 in = 2.5 cm Wt is measured in kilograms (kg) or pounds (lbs) 2.2lbs=1kg
Medical Implications Sudden change in wt within 24 hr period can indicate problems with circulation that can lead to cardiac problems. May need to measure extremities. EDEMA is extra fluid in the body tissues Some pt on chemotherapy will lose wt Anorexic or bulimic pts will be monitoring wt Diabetics often lose wt before diagnosis
Some arthritis or osteoporosis pts will decrease in ht Are children developing according to guidelines Is a pregnancy proceeding normally. Too fast wt gain could indicate mothers health problems
