Record subject eligibility Confirming a subject’s eligibility is not required by OnCore prior to placing that subject On Study Subject eligibility can be recorded directly in OnCore (when an online questionnaire is not being used for the protocol). Record Subject Eligibility Non-Cancer (MZ) How to record subject eligibility Navigate to Subject Console > Eligibility Click Update to begin entering data: Enter the following fields: Version Date Enter the version date of the eligibility questionnaire. Verified By Enter the name/initials of the verifying person. Status Date The date the subject completed the eligibility questionnaire and became eligible. Eligibility Status Enter the status based on review of the eligibility questionnaire. Status options are: Eligible - the Reason Withdrawn field will become enabled and selections can be made from a pre-defined list Not Eligible – if selected, the Reason Not Eligible field is enabled and a reason for ineligibility can be selected from a pre-defined list Eligible (O) - used when the subject has been found ineligible but the ineligibility has been overridden Comments Comments regarding this subject’s eligibility Click Submit The subject’s status will update accordingly.
