ROAD TRAFFIC FATALITY 5th March 2001 2 Dead Maqbool Abdulrasool al-Zadjali 28 years old Foreman Married Father of 3 yr old child Faisal Qasim al-Baluchi 21 years old Roustabout Not Married Head of family of 7 SEQUENCE OF EVENTS On Sunday 04/03/01 at 1800Hrs Maqbool and Faisal decided to leave WPH13 camp in Yibal, without permission, and go home to Muscat for the EID celebrations. They secured a lift from one of their colleagues to Nizwa and continued to Muscat by taxi. Their aim was to pick up a car (Jeep Cherokee) belonging to another colleague and return to Yibal in time for work starting at 1300Hrs on Monday 05/03/01. They left Muscat at approximately 0930 Hrs and at 1130Hrs were on the Nizwa-Izz section of the highway when they sustained a blow-out to the offside rear wheel of the vehicle. The vehicle went off the road on the right and into the ditch, they slid for approximately 40 metres then somersaulted four times coming to rest 146 metres from entering the ditch. Both pronounced dead at scene. 1

RTA Double Fatality 5/March/2001 Faisal and Maqbool were returning from Muscat to Yibal. An unauthorised journey in a private car. They had gone home for Eid. They were tired and had hardly slept. They were in a hurry. A straight road (near Nizwa): no traffic, 11.30 am. IMMEDIATE CAUSE(s) 1 High speed, from investigation speed was in excess of 150KPH. 2 Failure to wear seat belts. UNDERLYING CAUSE(s) 1 Unauthorised trip. 2 Urge to be with family during Eid celebrations. 3 Late in leaving Muscat. (Not able to make 1300Hrs start time) 4 Disregard for Omani Law (Speed Limit, Wearing of Seat Belt) 5 Failure to recognise hazards 2

RTA Double Fatality 5/March/2001 They were driving very very fast ! (150 km/hr??) This tyre burst. The driver lost control. The car rolled many times. Anything that goes wrong at high speed is a BIG problem!! 3

RTA Double Fatality 5/March/2001 Maqbool and Faisal were thrown out of the car and DIED on the spot. They were NOT wearing their seat belts. NO seatbelt + Rollover = Death!! 4

RTA Double Fatality 5/March/2001 Maqbool and Faisal were our colleagues They were travelling back from Eid celebrations. Their family will not see them again. Do NOT make the same mistakes. 6
