Active Lives: Children and Young People Survey
Why should you participate? Allows Sport England to gather physical activity levels for each Local Authority across the whole of England. For participating in the survey your school will receive a £100 incentive. Access to a detailed (anonymous) report on Physical Activity and Health and Wellbeing based on results from your students. Support for reporting impact of the Primary PE & Sport premium.
Once you agree to participate… You will be required to help select 3 classes that will complete the confidential online survey, these classes will be in three different year groups that have been randomly selected by Sport England. (classes cannot be chosen at will). You will receive a checklist and a FAQ document to help guide you through the process.
How the classes are selected… Single Form Entry – We would select the class in the year groups selected and work with that class. Multiple Form Entry – We would ask you to send us the class codes/ names that you use to identify the classes and use the tools on our end to randomly select one of these classes. Primary Mixed Year classes- If your school has mixed classes 1-2, 3-4 and 5-6. We will randomly select one of these classes and all children in that class will complete the survey. Primary Mixed and Single Year classes – If your school has a mixture of both mixed year classes and single year classes then we will select from the list of the classes that only contain the children from the selected year groups. (For example, Year 1 is the selected year group and your school has two year 1/2 classes and one year 1 only class, we will select the year 1 only class to complete the survey.) Secondary School – Doesn’t have to be completed in PE groups, it can be completed during Tutor Time, PSHE, IT lesson or any other subject that has a mixture of male and female students with a mixed range of ability.
Once the classes have been selected… We will send you a template letter which will need to be sent on to the parents/guardians of the children in the selected class. The template will give full details about the survey and give the parent(s)/guardian(s) the chance to opt out if they so choose. NOTE: Schools are to keep a note of students who do not take part in the survey for further reference, however we do not need to be notified.
Key Stage One Classes If we have selected a class in Key Stage One to complete the survey then we also ask that parents complete a short survey to help provide supplement to the answer provided by their child/children. We will provide you with a second template for a letter which holds the same information and ask parents to participate. NOTE: For every 5 parents that complete the survey, your school will receive £50 and could benefit up to £300 additional income. (£400 in total) Extra incentive amount Baseline (all schools) - 5 parent responses £50 10 parent responses £100 15 parent responses £150 20 parent responses £200 25 parent responses £250 30 parent responses £300
Setting a date… Once letters have been sent, we would ask you to set a date to complete the survey. All survey’s do not need to be completed at the same time, they can be spread across several days in order to fit in to your timetable. The deadline for ALL survey’s to be completed is Friday 14th December 2018. We would suggest you try to set a date ASAP and at the very latest, 2 weeks prior to the deadline in case any issues arise. This will give us time to try and resolve it. We will also send you practice links to check that there aren’t any problems gaining access to the survey.
Once a date has been set… Once a date has been set, you will receive your class URLs. You will receive 3 URLs for each individual class that have been selected. We would also require one teacher to complete the survey and will provide a separate link for them to use. If you have Key Stage One completing the survey you will receive another link which will need to be sent out to parents. (Coffee Morning’s, Parents Evenings etc are a great way to gather this data!)
Once you receive your URLs… Now that you have your unique URLs you are all set to complete the survey. Children will need to complete the survey in one sitting and it should take roughly 20 minutes to complete. If a student accidently closes it, they may restart unless they have passed the first section which will then mean their results have been recorded. Teachers will receive a URN alongside their link this will allow them to come back to the survey and complete it at any point.
Once the survey is completed… Upon completion of the survey you will receive a BACs form. This form will need to be completed and sent via post to Sport England. Accompanying the form you will be required to send an up to date bank statement which contains the schools bank, school details etc. on. However, any payments and balances can be removed. This is for security reasons.
Contact Details… Please can you confirm your school’s participation, the date you plan to do the survey and the person at the school that we should contact by contacting: jayne.thacker@livingsport.co.uk.
