5/10 Immediate implantation in molar site with AnyRidge 5/10 implant and 10/5 standard healing abutment. A case report. Kristina Saarepera DD, MSc Object Immediate implantation is an excellent way to maintain ridge shape after tooth removal. Sealing the socket is a known prerequisite for fast healing. In molar sites, immediate loading is not always an option due to several reasons, such as risk of overloading or complexity of the procedure. The aim of this case report is to present the possibility of using large prefabricated healing abutment for closing the socket of a molar tooth without additional augmentative procedures. Materials and Methods Pt: F/47yrs, generally healthy. D46 was extracted due to root caries.AnyRidge® ø5/10mm implant was inserted into septum after initial osteotomy with 2,0-2,8 drills and expansion with osteotomes (Nentwig, Ustomed). Primary stability 45Ncm, ISQ75. No grafting of the jumping distance was made. Healing abutment ø10/05mmwas used for sealing the socket. Supportive sutures were placed for closing the papillas. Results Pt had no postoperative problems such as swelling or pain. After uneventful healing of 8 weeks impression was taken and screw-retained monolithic zirconium crown was fabricated and inserted. The ridge shape was perfectly preserved only by supporting with the large healing abutment. No grafting of the socket was necessary to avoid the tissue collapse. Conclusions If immediate loading is not possible or desired, large standard healing abutment can support the tissues from collapsing and preserve the dimentions of the alveolar ridge even without socket grafting. Prosthetically, such method results in excellently emerging crown, as the form is created by standard gingiva former during healing phase, making treatment faster and more cost effective.
