TUR stage: bladder cancer stage at transurethral resection (TUR), where 1 denotes Ta, Tis, and T1 stages; 2 denotes stage T2; and 3 denotes stage T3 transitional cell carcinoma, according to the 2002 TNM classification. TUR grade: bladder cancer grade at TUR, where 1 denotes grades 1 and 2, and 2 denotes grade 3 disease, according to the 1973 WHO classification. Preoperative CIS: concomitant carcinoma in situ at TUR, where 1 denotes presence and 0 denotes absence; patients with stage Tis should be classified as 0. Instructions: To obtain nomogram predicted probability of pathologic T3–4 stage at cystectomy (A), locate patient values on each axis. Draw a vertical line to the “Points” axis to determine how many points are attributed for each variable value. Sum the points for all variables. Locate the sum on the “Total Points” line to determine the individual probability of pathologic T3–4 stage at cystectomy “Prob of pT3+” line. Use the same approach to obtain the probability of pathologic N1–3 stage at cystectomy “Prob of pN+” (B).
