Presentation transcript:

MULTIPLE FATALITIES 5th April 2008 Three contractors died with 58 injured, 31 seriously

A heavy duty bus approached a crossroads travelling at approximately 53kph on a graded road at the same time as a HGV tipper approached from the left Simple! What could go wrong ???

Discuss in your team: What could go wrong in this operation ? Any hazards/dangers? What if …….? Any controls needed? How would you minimise the risk?

* The bus drove across the junction without stopping at the STOP sign. What went wrong? * The bus drove across the junction without stopping at the STOP sign.

Discuss in your team: As a Driver: - How will you prevent this accident happening to you? - What will you not do which will prevent this happening to you?

Some other issues to consider: Why do you think the driver chose not to stop? Was he under pressure to reach his destination? Was he aware of the consequences of his activity? Was he competent to drive the bus safely? Do you check all of your passengers are wearing seatbelts? Do these issues add to the risk ? How do you combat that increased risk?

These three people would be among us if these rules had been followed Learnings: These fatalities occurred as a result of Not stopping at the mandatory STOP sign Failure to adhere to Defensive Driving Techniques STOP Intervention being ineffective Ineffective resolution of unsafe conditions by Contractors/PDO These three people would be among us if these rules had been followed

Learnings for you from the incident…….. Ensure everyone in the vehicle always wears seatbelts Always obey traffic laws and STOP at junctions Adhere to Signage on the road Always drive at speeds safe for the conditions Do not be pressured into rushing a journey Have you any more ideas for preventing this incident happening again?

Some more serious reminders….from 2007
