Length of Stay Reduction Project Goal : Engage in a collaborative person-centered process that will enable individuals with a hospitalization greater than 6 months to move into selected community residential settings.

Bridgeway Transitional Team Services (BTT) Team concept including consumer involvement; Peer Bridger services Increased emphasis on individual vs. group work Commitment to know and deeply seek to understand the individual Crossed systems Flexibility, innovation, and unconventional solutions Person-Centered Planning Tools

LOS Reduction Program Individuals with LOS greater than 1 year = 42 Individuals eligible for Bridgeway Transitional Services= 42 Individuals served by Bridgeway Transitional Services= 37 Individuals discharged from inpatient = 15 Individuals having re-admissions to inpatient= 2

John Over two years on inpatient unit Anger issues Multiple disabilities; wheelchair bound Evicted from multiple apartments Refused by multiple care providers, residential providers Permanently banned from local bus system

Johns outcomes as a result of person-centered practices Obtained housing Full array of outpatient services, supports Crossed systems, MH and OPWDD Renewed relationship with his family Made gradual progress dealing with anger Has succeeded in the community for 10 months post discharge

Future plans for Bridgeway Transitional Team Services Open a community residence/beds Person –centered training for residential staff Continue to work with long stay individuals on inpatient units toward community re-entry Create more opportunities for individuals to practice person- centered planning tools Develop new community connections Assist individuals to explore and experience a variety of community settings
