Extramedullary Multiple Myeloma in the Head and Neck: A Pictorial Essay  Michael Chan, BHSc, Eric Bartlett, MD, Eugene Yu, MD  Canadian Association of Radiologists Journal  Volume 64, Issue 4, Pages 363-369 (November 2013) DOI: 10.1016/j.carj.2012.08.002 Copyright © 2013 Canadian Association of Radiologists Terms and Conditions

Figure 1 (A, B) Coronal and (C-E) axial unenhanced T1-weighted images, and (F-I) axial contrast-enhanced T1-weighted images, demonstrating the presence of a mass lesion that involves the right masticator space. There is also infiltration of the floor of the right middle cranial fossa. Tumour is also present within the right sphenoid sinus. (J-K) Contrast-enhanced axial computed tomographic imaging, showing abnormal infiltration into the thyroid cartilage ala. Figure 2 (A) Axial bone algorithm image of the skull base, showing a lytic lesion within the clivus. (B) A coronal computed tomographic image showing a soft tissue mass located inferiorly in the left orbit in intimate contact with the inferior rectus musculature. (C, D) Axial postgadolinium-enhanced imaging demonstrating enhancing tissue in the clivus, left orbit, as well as a mass that involves the right greater sphenoid wing. Figure 3 (A-D) Contrast computed tomography study, showing a lobulated mass involving the base of the tongue, as well as right neck adenopathy in levels II and III. (E) Hematoxylin and eosin stained slide from a tongue biopsy (100× magnification) demonstrating atypical plasma cells beneath squamous epithelium. (F) KAPPA staining (100× magnification) revealed uniform positivity consistent with a monoclonal plasma cell proliferation. This figure is available in colour online at http://carjonline.org/. Figure 4 (A-D) Contrast computed tomography (CT) study in the axial and (E) sagittal planes showing the presence of a circumferential mass in the subglottic larynx seen as a soft tissue density filling the subglottis and extending into the proximal trachea. There is also evidence of cricoid erosion. (E) Sagittal contrast CT shows abnormal increased soft tissue density filling the subglottis. Figure 5 (A-C) Gadolinium-enhanced axial and (D, E) coronal T1-weighted images, and (F) an axial T2-weighted image, showing the presence of abnormal thickening and enhancement of the right cavernous sinus, as well as a contiguous mass within the right sphenoid sinus. Canadian Association of Radiologists Journal 2013 64, 363-369DOI: (10.1016/j.carj.2012.08.002) Copyright © 2013 Canadian Association of Radiologists Terms and Conditions

Figure 6 (A) Unenhanced axial T2-weighted, and (B,C) post-gadolinium enhanced coronal and (D,E) axial imaging, showing a large mass with the epicentre in the left maxillary sinus. The expansile mass has breached the walls of the sinus to infiltrate into the premaxillary and retroantral areas, as well as the adjacent nasal cavity. (B,C) There was also a second focus of disease within the left frontal sinus. (F) Hematoxylin and eosin stained slide (200× magnification) showing an abundance of abnormal atypical plasma cells. Immunohistochemistry showed features consistent with plasma cells given the positive reaction to CD138, MIB-1 and nonreactivity towards CD45, CD20, EBER, and keratin.
