February 23, 2017 Overall accident trend is down from 2008 However recently accident totals are on the rise 2017: 24 to date with 8 being FSI – 2017 Part 135: 5 and 3 February 23, 2017

Factors also include improper installation (2), and one each of bird strike, carburetor ice, cg out of limits, hard landing, improper maintenance, mismanaged controls, over gross weight, stolen aircraft, and system failure. 2016 Factors are: Pilot induced error (10), lost power (2), weather (1), and component failure (1). February 23, 2017 No discernable long term trend But there is a recent trend since 2014 of FSI Events increasing with a commensurate increase in fatalities and serious injuries FY 2016 Part 135 Operator Accidents: 18 of 91 total Alaskan accidents (all FAR parts) 7 of 17 total FSI accidents 7 of 16 total fatalities occurring during all accidents 11 of 18 total serious injuries occurring during all accidents February 23, 2017

From Fiscal Years 2008 – 2016, most accidents occurred during the landing phase of flight. However, in 2016, accidents in the cruise phase of flight surpassed those in the landing phase of flight by a count of 8 to 3. FY 2016 Part 135 accident count: Cruise (8), landing (3), takeoff (3), tax/ground (3), and emergency landing (1). FY 2008 – 2016 Sum (total amount of serious injuries and fatalities:) Cruise: Fatal (50), Serious injury (34) Takeoff: Fatal (12), Serious injury (11) Emergency landing: Fatal (6), Serious injury (10) Landing: Fatal (2), Serious injury (2) Hover: Fatal (1), Serious injury (0) February 23, 2017

Most FSI accidents occur during the cruise phase of flight. FY 2016 FSI accident count: Cruise (6), takeoff (1) Of the 29 FSI accidents during the cruise phase of flight from FY 2008 – 2016, 18 are CFIT accidents (including those noted as probable and possible) Of the 6 FSI accidents during the cruise phase of flight in FY 2016, 5 are CFIT accidents (including those noted as probable and possible) FY 2008 – 2016 Sum (total amount of serious injuries and fatalities:) Cruise: Fatal (50), Serious injury (34) Takeoff: Fatal (12), Serious injury (11) Emergency landing: Fatal (6), Serious injury (10) Landing: Fatal (2), Serious injury (2) Hover: Fatal (1), Serious injury (0) February 23, 2017
