Enhance Service Design and Delivery 12/3/2018 Enhance Service Design and Delivery Residential Supports and Community Integration Regional Stakeholder Advisory Groups oversaw RFA development for each region for the allocation of $10 Million in residential supports for those living at home. Over 25 Regional Stakeholder Advisory Groups meetings were held across the state. Two Request for New Services applications have been submitted and several more are anticipated. Overall, OPWDD’s Integrated Supportive Housing program expanded by 210 units in 2016, and now totals 336 housing opportunities statewide serving more than 355 people.   New prioritization criteria were issued for residential supports to ensure equal access for those living at home. Broome Development Center closed on 3/31/16. In 2016, 128 people moved into the community and now less than 280 people currently live in Developmental Centers. During 2016, 41 ICFs converted to HCBS community-based residential settings.

Enhance Service Design and Delivery 12/3/2018 Enhance Service Design and Delivery Employment As of 1/1/16, 7,958 people were competitively employed, and today 8,045 people are competitively employed. An additional 1,595 people enrolled in Pathways to Employment this year. Guidance was provided to sheltered workshop providers on conversion to integrated settings. SEMP providers participated in Communities of Practice to address best practices in employment and confront rate challenges. EmployAbility toolkit was developed to educate businesses on the benefits of hiring people with disabilities.

Enhance Service Design and Delivery 12/3/2018 Enhance Service Design and Delivery Self-Direction On 1/1/16, 7,952 people self-directed their services. As of 9/30/16, 10,682 people self-directed their services. An additional 5,000 people were provided education in Self-Direction between 1/1/16 and 9/30/16. Issued detailed self-direction guidance for service providers. Enhanced training courses for self-direction support staff. Updated and improved website content related to self-direction. Developed guidance regarding live-in caregivers. Initiated quarterly conferences with agencies providing self-direction support.

Enhance Service Design and Delivery 12/3/2018 Enhance Service Design and Delivery HCBS Statewide Transition Plan was created. All residences and day habs reviewed for Heightened Scrutiny triggers 2015-2016 (6,882).  All residences triggered (192) were reviewed for HCBS settings characteristics compliance. New DQI survey protocols include review for Person-Centered Planning (Person-Centered Review) and for HCBS compliance during Site Reviews. Launched Welcome community inclusion campaign.

Maintain and Strengthen the Workforce 12/3/2018 Maintain and Strengthen the Workforce Hiring process has been streamlined. PROMOTE trainings launched in State Operations statewide. 5,800 frontline supervisors, DSPs, service recipients and family members have been trained by Regional Centers for Workforce Transformation.

12/3/2018 Support High Needs Individuals in Existing and New Service Environments Over 2,300 CAS assessments have been completed. Negotiations underway for development of acuity- based rate enhancement for high needs individuals. Learning Institute established. START model is being expanded to downstate regions: RFA for Long Island was released on 10/21/16. START Resource Centers are under development in upstate regions: Region 1: Renovations will be completed early 2017. Region 4: Early phase of development; as staff are trained, cases are beginning work.

Ensure Provider Stability 12/3/2018 Ensure Provider Stability People First Community Fund will make $50M available to providers to ease rate transformation. Phase 1 funds have been distributed to providers, and surplus/deficit and award calculations are in final stages for phase 2. Interviews held with providers to gain input on needed rate rationalization changes. Workgroup for regulatory review with provider participation established to identify burdensome or obsolete regulations. Regulatory development process modified to allow for greater stakeholder input and schedule of rate changes posted on DOH website.

Implement Managed Care/Care Coordination/Value Based Payments 12/3/2018 Implement Managed Care/Care Coordination/Value Based Payments FIDA-IDD demonstration launched in downstate area. Care Coordination Organization structure developed as pathway to Managed Care. Six internal workgroups and a provider group are now meeting. Joint Advisory Council representing a broad range of stakeholders will be providing consultation on Care Coordination Organizations design. Value-Based Payment Pilot with Centers of Excellence for Children’s Residential Programs is developed.

Build and Maintain Foundational System Elements 12/3/2018 Build and Maintain Foundational System Elements Medicaid Waiver agreement was approved giving New York State the authority to provide Home and Community-Based Services to individuals with developmental disabilities.  Public dashboard posted indicating the number of services being provided. IBR study findings include: Brains of children with autism have neurons that are significantly smaller than normal, but brains of teenagers and adults with autism have neuronal volumes that are close to normal. Deep brain stimulation was found to be safe and effective in an experimental model of epilepsy.
