Communicate all did not pass results to the PCP/Medical Home as a critical value requiring confirmation

Increase physician knowledge of ALL hearing screening results Marathon County Plan: Develop an EMR report that will be readily accessible to all physicians (in the system and outside) electronically or as a hard copy. It will include newborn info. including the Hearing Screen results. Do: A report was generated that pulled all the relevant information from the EMR into one document. Study: PCPs were informed of NBHS results before 1 wk well child exam 100% of the time. N=63 Act: Continue to sustain these improvements and spread the good news to others in the state.

Increase physician knowledge of ALL hearing screening results Marathon County Plan: Treat the DNP as a critical value by calling the physician and reporting the results Do: Tested the use of a call to PCP. The Birthing Center RN called the PCP or PCP's Clinic RN. The call was to be documented in the medical records. Study: During testing it became clear that the results could not always be given directly to the PCP as sometimes they saw the covering physician. New protocol was tested that included making the covering MD aware via phone and follow-up with written documentation. Act: The treatment of the DNP as a critical value has been fully implemented
