ACT Ozark Coalition

ACT Ozark Started as a community collaborative with 15 organizations. We became an “official” Coalition in May 2016. Over 194 members with average quarterly attendance of 60. Three areas of focus.

Tracking Readmission Rates

Benefits of Coalition Participation Expanded resources for your patients. Guides a “shared goal” for the healthcare community. Fosters integration between community providers. Builds synergy through idea sharing. Great networking opportunity (for more than just care transitions)
