Regional TRACKER - SUMMARY Unallowable limitations or restrictions (i.e. facility subsets, system component exclusions) Example 1: A provider may have indicated in their “Explanation for selecting Measure Bundle” response that they intend to limit Measure Bundle C1 to certain primary care clinics within the measure bundle. Facility subsets are explicitly prohibited in the PFM, and HHSC’s approval of the provider’s measure selection does not constitute approval to use a facility subset. Example 2: A hospital provider may have listed certain services that are included in their inpatient services, but not listed all inpatient services. HHSC has made an effort to review services listed on provider websites, but cannot know each unit of service within a system component. Providers may not exclude services or populations from the required components of Category B system definition or Category B PPP. If a provider has omitted certain services or populations and needs to revise the system definition or PPP, please do so in the appropriate “Provider Response” column. Regional TRACKER - SUMMARY

Regional TRACKER - SUMMARY HHSC identified very few issues related to appropriateness of measure selection. The majority of exception requests did not include sufficient details to allow HHSC to evaluate the appropriateness of the request. Where additional details are requested, providers should include concrete and measure specific justification for exception requests through the NMI response (anticipated denominator volume, dates of data changes) Achievement milestone exemption requests and Reporting milestone exemption requests were often confused. Providers should review the RHP Plan Update Companion for a detailed description of each milestone exemption request. Regional TRACKER - SUMMARY

HHSC cannot approve a request to use the all-payer population for a Category C goal achievement milestone solely because the MLIU population is a small percentage of the provider’s total population. A significant MLIU denominator is 30 or more denominator cases for most measures. The target population of the DSRIP program is individuals with Medicaid coverage or who are low income without a payer source. Category C is not reimbursement for services, and HHSC supports providing quality care to all individuals seen by DSRIP Performing Providers, including those that do not meet the denominator inclusion criteria for selected measures or for goal setting and achievement. HHSC is still accepting requests for approximate baselines.
