Welcome to The Stratford! We have invited you because of your commitment to the clients we all serve and wanted you to learn about a program we are offering. We are really excited! 12/4/2018
Introducing Passages We are embarking on a special program called “Passages.” Program specifically designed for our assisted living residents living with varying stages of dementia. These residents are not yet ready for a secured, memory care environment, although they having difficulty “fitting in.” 12/4/2018
Common Symptoms/Behaviors Anxiety Depression Low self-esteem Reclusive behavior Stunted brain functions 12/4/2018
Anxiety This can stem from an awareness of their short-term memory loss. They are afraid of forgetting things or missing out on important, or even simple events. May be due to not being sure of “what to do next.” 12/4/2018
Depression So common in many of our elderly, but especially in cases of dementia. Often accompanies the leaving of “home” and all things familiar. Manifests when their independence is seemingly stripped away and they lack the ability to understand why. 12/4/2018
Low Self-esteem Comes into play when they are in a group with non-dementia residents whose minds are still sharp. Feelings of being inadequate or unsuccessful are extremely common. Other residents reactions can add to these feelings. 12/4/2018
Becoming Reclusive Find themselves in their apartments and coming out only for meals. Difficulty “fitting in” so they chose not to participate. Others may just sit, taking no initiative on their own. 12/4/2018
Decreased Brain Function Studies continue to show the importance of the “use it or lose it” philosophy, even as it relates to brain function. Providing people with the motivation, the setting, and the activities, helps keep the brain more active, for longer and ultimately…... 12/4/2018
SUCCESS! Passages provides these residents with a “safe” environment where they are placed in small groups of people with similar struggles. This aids in eliminating the intimidation and inadequacy often felt when grouped with higher functioning and non-dementia residents. The small group size lends itself to more one-on-one involvement. 12/4/2018
Passages…. A large part of this program is to provide the dementia residents with the daily “brain” exercises necessary to increase and maintain a higher functioning status. 12/4/2018
Passages Residents will…… Be successful in small group activities and they will be more relaxed, more confident and integrate more comfortably when combined in activities with other non-dementia residents 12/4/2018
Small group size…….. Lends itself to more one-on-one involvement and participants enjoy a structured activity program that begins right after breakfast and continues throughout the day. Great care is taken to provide activities that will be mentally stimulating. 12/4/2018
The Passages Group meets… Alone for part of the day. Integrates with the regular programming, when appropriate and between their sessions. Participants are “directed” and strongly encouraged to participate all activities, thus filling the down time the residents struggle to fill. 12/4/2018
Goals: To increase and maintain brain function. To build self-esteem and confidence. To relieve anxiety. To build a sense of family and community. 12/4/2018

Whew! That’s a very full day. Day is structured to promote ending an activity on a positive note, while the resident is still feeling success. Not all these activities will be completed; some may not even be offered that day. The goal is to make sure they are planned for, so that as the residents move through their day, the coordinator is prepared to engage them in interesting and beneficial activities. 12/4/2018
If another opportunity arises, we can seize the opportunity! Improvisation is good! The fluidity of the program allows us to change the schedule quickly, if residents are especially enjoying one activity or another. If another opportunity arises, we can seize the opportunity! 12/4/2018
Passages Residents… Will also be included in the activity schedule that is offered to the community as a whole. Socials, parties, musical and other group activities will offer the residents opportunities to interact with other residents, while still receiving guidance and positive reinforcement from the Program Coordinator. 12/4/2018
Passages Program Will engage a coordinator specifically to run the program and provide a structured schedule as well as make sure the resident attends all of the activities. Passages will have designated staff members that are trained to run the program 7 days a week. 12/4/2018
