Athletic Forms All UIL and CISD forms are located Rankone Sport. Follow the link on the football website. CISD Policy states NO athlete will participate in any practice or event until all forms are complete and turned in to the athletic department. Physicals will be stored in the training room.

Athletic Physical CISD requires a current physical exam dated after April 1, 2018 Make sure physical and medical history is complete and signed by the physician, parent, and athlete. Make sure that any question marked “yes” on medical history portion is explained in the appropriate area.

Hydration Drink lots of water. Avoid sodas, tea, coffee, high sugar (corn syrup) drinks. Athletes must replace fluids lost through sweating (weight chart) before returning to practice. Replace electrolytes (sodium, potassium, calcium) through diet.

Asthma Athletes use inhalers AS PRESCRIBED Parents should provide an inhaler that will be stored in a secure area yet accessible for all practices and events Students who need a rescue inhaler should keep one in the training room and the nurse’s office

Concussions According to HB2038, a concussion is defined as a temporary impairment of brain function. It is an injury to the brain, and must have time to heal like any other injury. The terms “bell ringer” and “dinger” no longer apply. CISD has a Return to Play concussion protocol for all CISD athletes.
