RED ALERT On the morning of 18/06/2018 a container door became detached whilst the container was being loaded from empty. Only minutes before the incident, the Shunter Driver had opened the door and could have been crushed by the door if it fallen at that point. The door hinges had corroded through and this was difficult for the Shunter driver to see. Fortunately there were no injuries, but there was potential for significant injury Learning Points: Be Aware of the potential for container doors to become detached and fall Always check container doors for signs of damage and corrosion, particularly around hinges and fasteners before opening Always open doors in an area with plenty of space to retreat Whilst opening and closing doors, watch out for any signs of the door drooping down Stop and Report any concerns with the security of container doors Date of Incident: June 2018 Severity: Near Miss
