DIS ID Cards (Hang Tags) Permanent Temporary
Permanent DIS ID Cards Ability to walk is impaired Authorization of health care specialist required Recertification every four years May have 2 cards, or, 1 card if disabled license plate (unless plate is for a motorcycle, then 2 cards) No fee for permanent cards (except counter service fee of $3.00)
Processing Never count the month you are in Give customer four years from next month Cards expire on last day of month RaTS will default to correct expiration Multiple cards should expire at same time Issue only 1 card unless customer specifies 2 in writing on application Can use information from cards to issue disabled plates Notate plate application with card information
Temporary DIS ID Cards Good for up to six months Never count the month you are in Cards expire last day of month System defaults to six months but can be changed if authorization is for less time Cannot be renewed May obtain authorization for additional temporary cards Fee is $6.00 (plus $3.00 counter service fee)
No Fee 1 + 1 Temporary Cards Cards expire last day of next month Override fee: no counter service fee Use override fee code 15 (Temporary Allowable Short Fee) May be issued to: Customers with an obvious impairment and need to get application completed Out of state residents with proof of parking privilege but forgot card Out of state residents with impairment that occurred here in WI Customers with renewal of a permanent card but you don’t have the correct stickers
Give to customer: Issuance Renewal
Customer should leave with: DIS ID card Expiration stickers – month and year Copy of application with medical authorization completed Certificate of Registration (also shows fees) MV2752 – Important Information For Use of Disabled Parking Identification Cards
