About the Project “UNIversal solutions in TElemedicine Deployment for European HEALTH care” Coordinator: NHS 24/Scotland Geographies: 14 regions in Czech Republic, Finland, France, Germany, Greece, Italy, Norway, Slovenia, Spain, and UK 19 deployment sites Key facts: Three years (2013-2015) > 12K patients in 14 pilot sites > €10m (of which €5m EU funding) Four chronic conditions 12/5/2018

Ambition A study of large scale deployment, with assessment of: organisational aspects efficiency gains economic aspects Deliver evidence of health outcomes Commitment of regions to further deploy and promote telehealth after end of project 12/5/2018

Chronic Disease Management A common design for disease management with telehealth across 19 deployment sites from 10 countries for patients living with diabetes chronic obstructive pulmonary disease (COPD) congestive heart failure (CHF) hypertension 12/5/2018

Key accomplishments to date Care pathways aligned for diabetes, COPD and CHF 14 regions/19 sites deployed interventions with more than 10,000 patients Project is now gathering learnings and evidence, and developing key policy messages All deployment sites committed to further deploying Final project conference with session in European Parliament on 1/2 December 2015 in Brussels 12/5/2018
