Chronic Disease Management A common design for disease management with telehealth across 19 deployment sites from 10 countries for patients living with diabetes chronic obstructive pulmonary disease (COPD) congestive heart failure (CHF) hypertension 12/5/2018
Key accomplishments to date Care pathways aligned for diabetes, COPD and CHF 14 regions/19 sites deployed interventions with more than 10,000 patients Project is now gathering learnings and evidence, and developing key policy messages All deployment sites committed to further deploying Final project conference with session in European Parliament on 1/2 December 2015 in Brussels 12/5/2018
