The Facts Older adults are the fastest growing segment of our population They have a higher number of chronic diseases and are more vulnerable than ever before YET... Their needs are not well met by current models of health care

Creating a model of care where Teams are able to collaboratively assemble the "big picture" of each person's health story Frailty is at the forefront of evidence-informed decision making Patients and families feel empowered by information Optimal use of resources Improved navigation across the health care continuum

PATH programs were developed and trialed in response to the challenges of frailty

PATH Approach

Outcomes, 150 patients First 150 PATH, other facts 71 patients had a total of 77 procedures scheduled at the time of PATH consultation 75% of these were cancelled First 150 PATH, other facts 10% required hospitalization, but were cared for at home Survey showed very high patient and/or family satisfaction
