Primary Care Plus: Advanced Collaborative Perspective THANK YOU ALL We are still on the journey Patient perspective remains crucial and engagement the key We must bend the cost curve

Today we … Heard about shared decision making, further refinements in high risk identification, and offered a wide range of focused learning in the break out sessions Shared advanced thinking around approaches to integrated care along the age continuum (an important extension of the CTC mission) Offer a time for cross-fertilization across and within practices

Advanced Collaborative Perspective Advanced Primary Care is the most effective foundation for delivery system transformation A Multi-payer approach is the most effective and efficient way to: Deploy systems of care Leverage technology and common analytics Foster alignment between primary care, specialists, hospitals, employers, state govt, etc. An integrated approach to practice transformation

Advanced Collaborative Perspective The vision is for all practice to become members of the advanced collaborative as they finish the developmental contract. We continue to articulate a stronger vision of “advanced primary care” to include integrated screening and treatment for mental illness and substance abuse and the value of community health teams to more vulnerable populations. Details matter. How we do it is as important as what and why
