Receiving Inspection, Installation and Maintenance Strengthened content of clauses: Clause 4: Receiving Inspection Added inspection of all equipment and tools associated with the SSV/USV, such as SSV lock open devices/heat sensitive overrides. Clause 5: Installation and Maintenance Added requirement for SSV lock-open devices or manual override to be inspected in accordance with the manufacturer’s procedures, prior to installation. “Operator’s work procedures utilized with SSV/USV equipment shall: observe relevant information provided in the manufacturer’s operating manual.”   address cleanliness and condition of supply lines and control fluid.” Actuator supply medium: “clean” changed to “within manufacturer-specified and facility cleanliness requirements” At installation, valve shall be “operated several times” changed to “operated a minimum of three full open/close cycles” 12/7/2018

Clause 6: Repair Field repair: Offsite repair: Documentation will now follow safety equipment to maintain SSV/USV certification. (In original ballot) Only change from original ballot: “Field repair shall be accomplished by qualified personnel, and in accordance with OPD” Offsite repair: To maintain SSV/USV certification, equipment repaired offsite must adhere to 6A Annex J repair criteria and additional requirements imposed by 14H: RL level of SSV/USV must meet or exceed PSL requirements of the SSV/USV. Documentation requirements of repair shall meet or exceed RL 3 requirements, irrespective of the actual repaired RL. 12/7/2018

Clause 7: Testing 7.1 General Owner/operator written procedure shall be in compliance with government regulations, the requirements of section 7 of 14H, and should be performed in accordance with manufacturer procedures. Stipulates requirements that ensure meaningful test results are obtained. Conditions upstream of test cavity (pressure/temperature) are stable. Stability conditions modified. Pshut-in > 600psi: +/-5% or 3.45 MPa per hour Pshut-in <= 600psi: +/-30 psi per hour No grease/sealants are injected during testing. Imposed requirement for retention of periodic test records (created Table 4) 7.3.2.3: Added requirement for valve to be revalidated if SITP increases as a result of well stimulation or other production enhancing process. 7.3.3.k (cycling) and 7.3.3.l. (one additional pressure hold) apply only to field repaired valves. 12/7/2018

Failure Reporting and Documentation Requirements Clause 8: Failure reporting Added: One or more entities are involved in the maintenance and repair of these products in the field. The reporting forms consider the specific roles that are involved in the maintenance and repair of these products. When one entity executes multiple roles in this process, then that entity would be responsible for the information related to those roles in the failure report. Clause 9: Documentation Requirements: Requirement to “have access to or retain” documentation instead of requirement to retain documentation. Offsite repair/remanufacture records include traceability, test records, and certificates of conformance. Created Table 4 SSV/USV Functional Test Data Sheet For Periodic Testing 12/7/2018

Annex A After test, check added which adjusts allowable pressure based on actual test cavity temperature. Procedure: Step 1 can now be skipped if the GTL at actual conditions is known. Procedure: Incorporated calculation of actual leakage rate in the procedure. Example 2: Now shows calculation of actual leakage rate. 12/7/2018
