OUTREACH & ENROLLMENT PROGRAM Medicaid O&E OUTREACH & ENROLLMENT PROGRAM Enroll uninsured children and families into free or low-cost health insurance HealthFirst Colorado (Colorado’s Medicaid program) Child Health Plan Plus Kaiser Permanente’s Colorado Bridge Program Enroll families into SNAP food assistance and cash assistance Provide case management services!
