Our Innovation Project What is it? Transition from children’s to adult services Why did you choose it? Questions young people have expressed What’s its purpose? To give young people a voice regarding service development

Findings What impact do you think the project has had on the children and young people involved? They worked together as a group exploring their thoughts, feelings and wishes about moving to adult services. The emerging friendships within the group. What are you most proud of? The production of an animated DVD How did you as a Children’s Nurse benefit from leading the project? Greater understanding and appreciation of the needs of young people facing this transition. Sense i Sense of pride in their achievements

Next steps… What are your plans/hopes for the project for the future? The project better prepares the young people for the move to adult services. Adult services are better prepared to address the issues raised by the young people How would you like to develop it further? Ensuring the DVD is available to be used in other trusts with young people who are also dealing with epilepsy and other chronic conditions
