E-Contact Information During Exchange Form ● Complete the form and submit it within 1 month upon arriving at host institution To access the form: Please login to "MyCUHK" → "CUSIS and MyStudy" → "CUSIS Services" → "Contact during Exchange" to fill out your contact information during exchange

Verification of Full-Time Enrollment Form ● Complete this part after arriving at your host institution Submit this form within two months after arrival at host institution or right after course registration, whichever comes first To download the form: Home (www.oal.cuhk.edu.hk) > Study Abroad & Exchanges for CUHK Students > Downloads > Verification of full-time Enrollment form ● Ask the staff at the host institution to complete this part ● Email the completed form to OAL
