Figure 1 Response of the LES to esophageal lumen distension in achalasia. The patient had a reduction in basal LES pressure after pneumatic dilation, but remained symptomatic. The presence of a large, type III hernia allowed assessment of LES responses independent of crural contractions. With sequential water swallows that distend the esophageal lumen and raise intraluminal pressure, the low “resting” LES pressure is replaced by a hypertensive contractile response.
