CCG Innovation Bursary

Green Prescriptions - Social prescriptions linking people with high support needs Mapping Lack of capacity Lack of awareness of broader outcomes. In 2016 Tower Hamlets CCG rolled out social prescribing across its 8 GP Networks, that each now run a different model of patient support, and referral system to the wider community projects and services. However there have been only marginal numbers of referrals to ‘green’ projects that clearly should be part of a new solution for the health and social care system helping to reduce dema Unique model – social prescribers, wellbeing link workers, networking breakfasts, . Social prescribers are now contacting city farms and community gardens asking for money to set up their own gardening projects when numerous community groups are in need of referrals!! And asking the charity sector for funding! 7 7 7

Tower Hamlets Green Care Partnership CCG Innovation Bursary Borough wide vision linking nature and health Evidence based Delivered 16 Taster Sessions showcasing services for 87 providers and 34 referrals Working together to: Ensure Quality Safe, best practice service Demonstrate outcomes Streamline support We have created a partnership between 6 leading green care providers: Stepney, Spitalfields and Mudchute City farms, TH Cemetery Park that boast londons most urban woodland. Furry Tales – the only specialist animal assisted activity organisation working with people with dementia in nursing homes and hospitals. And recently the Bromley by Bow centre joined the partnership. None of these organisations have worked collaboratively before despite most being around since the late 1970’s. Collectively they have over 200 year of engaging local communities with nature, and represent 70 acres of community run green space in this densely populated urban borough, roughly one third of the size of Victoria park and a hugely important health resource within Tower Hamlets. 8 8 8
