الدروس المستفادة في مجال الصحة والسلامة والبيئة الحادثة: كان الطاقم يُحَمِّل حزماً من الأنابيب بواسطة معدة تحميل وتنزيل في منطقة تجميع الأنابيب. وكان من الصعب الوصول إلى بعض الأنابيب، مما استدعى جر إحدى الحزم، ولكن أدى ذلك إلى انزلاق الأنابيب، فاصطدمت برئيس الفريق وأدت إلى بتر ساقه. الدروس المستفادة: لمشغلي معدات التحميل والتنزيل عدم رفع أية حمولة بدون تخويل وإشراف مناسب. عدم رفع أية حمولة إلا بعد تلقي التدريب والحصول على شهادة اعتماد. عملية التحميل أو التنزيل محفوفة بالمخاطر، لذا يلزم الابتعاد عن منطقة الخطر. عدم تحميل المعدة فوق طاقتها. التأكد من عدم وجود أي أشخاص بالقرب من الحمولة قبل الشروع في العمل. التأكد من التدرّب على أحدث المستجدات. أنابيب منزلقة كان الرجل محبوسًا هنا أهم الدروس المستفادة: عملية الرفع خطرة – ابتعد تماماً عن موقع العمل بتر الساق Use this Alert: Discuss in Tool Box Talks and HSE Meetings  Distribute to contractors  Post on HSE Notice Boards  Include in site HSE Induction Contact: MSE511 for further information or visit the HSE Website Alert No 81 LTI#13 2012 July, 2012
