Developing a plan for Barnsley… Warwick Certificate in Diabetes Care Researched the evidence for Diabetes Education. One of many key documents- Diabetes UK (2016) Diabetes education: The big missed opportunity in diabetes care Attendance to XPERT was poor in Barnsley… Why? Audited attendance: The most significant issue was lack of information given to the patient on referral. The result: A Project that focused on the ‘Promotion of Diabetes Group Education’ Communicating with others was key! AUDIT & PROJECT Became familiar with the research for education. My audit highlighted poor attendance. key themes included= lack of accessibility but most prominent factor was lack of information given to the patient on referral. Direct audience to audit and Project… offer to send a copy this to individuals at the end. Lots more info/references to the evidence base. Much more detail into how I carried out the audit and project.

Networking is key Networked with Practice Nurse lead Gathered the evidence to deliver to colleagues in a concise clear way. Presentation delivered at PN Forum…. Contact with local businesses to improve accessibility: Barnsley College, Tesco, Barnsley FC, The Metrodome PN FORUM TALK – key points: deliver the benefits of education… both health and financially, raise awareness and improve PN confidence to positively promote education. Once we had them on side we needed to ensure that the education was accessible which is when we reached out further than our own room booking system to help broaden our ability to deliver education across the patch at various times

Promotion in Progress Following the presentation to the Practice nurses we have had lots of interest in XPERT from both Patients and Health professionals We’ve delivered the Education power point at various GPs across Barnsley. We have demonstrated the XPERT Kit during these meets as well as disseminating leaflets. Diabetes Education Champions Requests for our newly developed leaflet…. Also recently developed some short films for social media promotion! XPERT LEAFLET & STOP GAP notes- Contacted CCG’s across the country who were performing well with group education attendance. Asked if could take ideas from their promotional info then used their best bits to develop Barnsley’s trifold
