eSIS Special Education Module Referral for Evaluation (PR-06)

Evaluation Team Report (PR-06) SEWOH61 Allows for creation of Part A evaluation reports from each evaluator. In the future; The ETR summary will automatically be populated to “Present Level of Performance” on the IEP.

The Evaluation Team Reports Summary Screen provides a summary of the evaluations on record for a particular student. Lists Multi Factored Evaluations completed for a particular student. If a student had multiple evaluations completed, they would appear on this list. To view or modify a particular component, place the cursor to highlight the desired evaluation, then select the Part of the ETR Form to be viewed. When user select “Add ETR”, the Part B Form will automatically appear since a Chairperson must be assigned.

The “Chairperson” must be assigned and is the only person who can finalize the ETR. Districts set procedures for identifying chairpersons within their district to meet their needs. Once finalized, the ETR cannot be modified. This is built-in protection for the district which allows date/time stamp to be defensible.

This screen shot displays the box that will appear when user selects the [?] box next to a name field. This displays a staff list from which a user may “Select”. User can enter a specific name and press the “Re-query” button to locate particular teacher or scroll through the list to identify the particular teacher. The % sign in a query acts as a wild card to locate all records within the table.

Screen for viewing Part A Reports Screen for viewing Part A Reports. This is a summary form listing all the Part A Reports on file for the respective evaluation. The user can select the evaluator from the list, then select Part A to view the particular evaluator’s report.

Additional capability to attach third party software reports will be incorporated into future releases. This will be particularly helpful for psychologists or therapists who want to develop/utilize a template. The use of the attachment feature would allow evaluators to include as much information as they would like to include as it would not be subject to field size restrictions. Use of common software such as Adobe or Word would allow others to access documentation. Although other formats can be attached, they may not be compatible on all computers. The summary section will be used for printing to present levels of performance on IEP.

With updated Windows XP or with some internet browsers, user may get a pop-up block message when user attempts to preview depending upon the security settings on user computer. To allow the Adobe Form to display, user must allow the pop-up either temporarily or always for this site.

This is a sample of the Adobe version user will see as user select preview from the Evaluation Team Report (Part A) Screen. This example does not have data entered into the fields. As mentioned previously, information can be attached as a Adobe file to an email so that others may preview.
