The rank order of the histologic lesions from higher risk of mortality to lower risk of mortality in NAFLD is as follows: Fibrosis is the most important histologic lesion that is associated with long-term outcomes in NAFLD. Among fibrosis stages, it is the presence of advanced fibrosis including bridging (stage 3) and cirrhosis (stage 4) that has the highest risk of mortality, and this increased risk of mortality is not just for liver disease, but also for cardiovascular events. Fibrosis is followed by portal inflammation, the diagnosis of NASH, and presence of ballooning, respectively.
