NEWS Patient observation chart Aims to identify early deterioration Generates a score based on patients observations Provides escalation guidance dependant on score Scottish Patient Safety Programme

Stage 1 Initial audit of 100 post procedural patients NEWS charts to identify compliance Result – 58% completion

Stage 2 Daily safety brief – results highlighted to staff, daily reminder to ensure charts are completed 20 charts re-audited 70% compliance

Stage 3 Lack of equipment identified Equipment made available Re- audited 2 weeks – 75% completion

Stage 4 Poor documentation of respiration rate. All staff asked to complete NES’s NEWS e-learning module through TURAS. Re-audited after 2 weeks – 90% completion

Conclusion Improved record keeping Importance of patient observations and NEWS score highlighted through e-learning Staff awareness of the issue raised Continued monitoring of NEWS charts on a monthly basis
