HIV Cross-Part Care Continuum Collaborative Milestones Learning Session 2 Faculty Storyboard Consumer Training All states receive at least one training by LS3 Beyond 2015 NQC and HAB continue to provide nominal support Continued participation in PM, CC, and Cohort data Continued participation in QIP sharing Continued training and capacity building efforts Continued communication around H4C activities Learning Session 1 LS1 Storyboard IT Assessment Survey Completion State Letter of Commitment Response Team Formation QI Inventories –care continuum interventions implemented? Care Continuum Update – where is your state at with this?
