RISK ANALYSIS SECTION 6 – Risk Assessment Tool (Hazpak) Health & Safety Human Resource Services

RISK ANALYSIS – Hazpak Disclaimer: The use of risk assessment tools is subjective in nature and it must be clearly understood that a score indicating minimal or no action to be taken or high dollar rectification costs does not absolve management from its duty to provide a safe workplace.   Hazpak - What is it? Hazpak is a risk management tool designed by the WorkCover Authority of NSW to enable Supervisors, management and employees to work together to make the workplace a safer and healthier place. The Hazpak document is presented in worksheet form and very user friendly. Information recorded on the Hazpak worksheet can be easily transferred to a main hazard registry and action plan.

RISK ANALYSIS – Hazpak Using Hazpak   When using the Hazpak worksheet there are five steps to follow: 1. Record the/each observer's name and the date on the front sheet of the Hazpak tool. 2. Identify any hazards to health and safety that exist at your place of work and list them in the space provided. 3. Decide how big a risk each one is by using the easy to follow risk-rating matrix provided. Write down in the spaces provided how severe and how likely the hazard potentially is and then give the hazards a 'priority' risk rating. Document the risk rating in the space provided. 4. Work out what to do to eliminate or reduce each hazard and record the controls in the space provided. The order in which subsequent control actions are carried out should be in priority order. Higher priority hazards should be addressed first. 5. Make sure the solutions (controls) really do work. If they do not then start the process over again.

RISK ANALYSIS – Hazpak Hazpak Thinking Prompts The Hazpak provides convenient to use 'thinking prompts' when identifying hazards in the workplace. Laid out in checklist form the prompts, while not exhaustive, encourage one to consider: Things that could injure or make people sick Things that could go wrong with plant or systems of work Things that could cause injury or illness through bad workplace practices; How injuries might happen to people What dangers you would warn a ten-year-old child to be extra careful of should they come into your work area and What special hazards exist at your place of work (eg during maintenance or irregular work) ·By using Hazpak and writing down the hazard identification, assessment and control process you are demonstrating that the risk analysis process is being conducted in a systematic and legally compliant fashion. The completed document also provides a record for future reference when faced with similar hazards. Hazpak records can also be transferred to a central filing system called a Hazard Registry.
