Rural Health Clinic Practice Transformation Collaborative November 2013

Goal Increase RHCs understanding and capacity to implement medical home and practice transformation principles Position RHCs to improve care and participate in payment reform initiatives Prepare RHCs for successfully meeting future requirement to report on quality data measures

Collaboration CoOportunity Health Iowa Primary Care Association Funding Iowa Primary Care Association Certified NCQA PCMH content expertise; provide support and consulting to Telligen coaches Telligen NCQA trained coaches to deliver training & technical assistance Manage day to day work of the Collaborative Iowa Association of Rural Health Clinics Recruit RHCs and champion the effort

Strategy Cohort 1- up to 9 RHCs Cohort 2 – Available to all RHCs One-on-one guidance, 1-2 onsite meetings Regular teleconferences Tools and dashboard training Networking and sharing experiences Statewide learning session plus additional intensive consultative session Other items available to cohort 2 Cohort 2 – Available to all RHCs Monthly webinars, toolkits, self assessment Statewide learning session

Metrics Bi-monthly report done by coaches Amount of TA provided Level of clinic engagement Degree of progress & responsiveness Bi-monthly report done by RHCs Status of each change concept from RHC perspective Rate TA received (tools and coaches) Quality data measurement To be selected from recommended national measures for RHCs

Timeline November 13th; Informational Webinar for RHCs December; Selection of Cohort 1 April 2013; Statewide learning session plus additional intensive consultative session (coincide with IARHC spring meeting) December 2013 – October 2014; Education and coaching; Project ends December 2014; Final Project Report
