Lockout A technique used to prevent hazardous energy from escaping. A padlock is placed on a disconnect switch, circuit breaker, valve handle or other energy-isolating mechanism that is in the off or closed position.

Tag Out Energy isolating device is placed in the safe position and a written warning tag is attached. This warning tag must have the authorized users name and department.

Who Can Lockout Equipment? Only Employees who have been trained and authorized by management and have received the proper equipment

Authorized vs Affected Employees Personnel trained to lockout-tagout machines or equipment to perform servicing or maintenance on that machine or piece of equipment is the AUTHORIZED employee. The employee whose job requires operating or use of a machine or equipment on which servicing or maintenance is being performed is the AFFECTED personnel.

Use your own Lockout Tagout device Every single authorized employee working on a system shall independently lockout and/or tagout the system using his or her own lock and/or tag eliminates the possibility of others inadvertently unlocking an isolation device when it is being worked on by another. at no time should any employee depend on the lock or tag of another worker to protect them during their service activities.

First Step In LOTO 1. Know your equipment & systems hazards isolation points procedure for Lockout/Tagout Working on unfamiliar machinery is a hazard

Next Steps 2. Inform affected personnel Inform appropriate departments and employees in the work area 3. Prepare for shutdown Know energy types and hazards 4. Shut down Follow the guidelines of the machinery 5. Isolate equipment Turn off EVERY energy source feeding the equipment.

Next Steps 5. Apply lockout / tagout device At all disconnects, valves and energy isolating devices 6. Control STORED energy Relieve disconnect or restrain residual energy that COULD be present 7. Verify Isolation of equipment Dont take anything for granted. Double check !

The Fatal Five Failure to stop equipment Failure to disconnect from power source Failure to dissipate (bleed, neutralize) residual energy Accidental restarting of equipment Failure to clear work areas before restarting

Removing locks and tags 1.Make sure equipment is operationally intact 2.Remove tools and non-essentials 3.Tell all affected employees(head count) 4.Make sure area is clear 5.Remove lockout /tagout devices Each worker removes their own lock/tag 7.Tell affected employees locks and tags have been removed and equipment is ready to use

Lockout-Tagout Protects YOU! NO JOB IS SO IMPORTANT, NO TASK IS SO GREAT, THAT YOU CANNOT AFFORD TO TAKE THE TIME TO DO IT SAFELY
