Necessary Paperwork 2 copies of Physical Completed by a Doctor (dated after April 1) Athletic Participation Form (including insurance information) Birth Certificate Proof of Residency (not a cell phone bill) Report Card (from previous marking period) ImPACT/Concussion Forms Sportsware (Emergency Contact form – online) All paperwork must be submitted at the same time

Academic Eligibility A student must comply with the Academic Eligibility for Extracurricular Activities Policy(Policy #9070). Students must have earned a 2.0 GPA with no more than 1 failing mark (E) during the previous quarter. The grade entered on the report card determines eligibility, not a subsequent change (except in the case of teacher error).

Drug and Alcohol Policy All students must comply with the Drug and Alcohol Policy(#9230). Use and/or possession, intent to distribute and/or distribution of drugs/alcohol/non-controlled substances is in violation of this policy and will result in a 30 day school suspension from extracurricular activities. A copy of the policy is located in the school office. This includes the sharing of prescription medications
