Anti-TNF treatment of a complex fistula without seton drainage leaves the potential for premature closing of the internal and external fistula openings, creating an abscess cavity. Seton placement before anti-TNF treatment controls fistula healing, allowing the inflammation to be treated completely and the exudate drained before track closure.
