Workshop on Theme 2 "ICT for accessibility, ageing and social integration" opened in Call 1 of CIP ICT PSP programme Objective 2.2: ICT for ageing well Brussels, 25 May 2007

Who are we? Hospital Clinic of Barcelona Regional partners from Catalonia that work on ICT projects for healthcare and quality of life of the elderly people. Hospital Clinic of Barcelona ICT supported chronic disease management Focus on integrated healthcare model for the management of chronic patients -> Several R &D and validation projects ( FP5, FP6 and eTen) Foundation of the City of Viladecans (Barcelona) ICT integrated care for frail older chronic patient Focus on integrated social and health care at local level -> Interreg 3C project + Projects on educational tools for the empowerment of the patients and family carers Close cooperation with key regional partners for replication purposes: - Health department of Catalonia - Welfare department of Catalonia - Social services of local authorities

Current projects related to call 1, obj. 2.2. Hospital Clinic de Barcelona - Application and testing of mobile solutions (using GSM, GPRS, UMTS) to specific programmes of care delivery (home-based hospitalisation, prevention, palliative support…) for chronic patients. - Application and testing of innovative front-end systems for patients and professionals (based on PDAs, TabletPCs, Mobile phones), to support specific actions in the context of the care programmes. - Development, validation and deployment of an integrated care model for the management of chronic patients based on a network of coordinated actors across providers and levels of healthcare. (eTen projects: Linkcare, Healthservice24) - Together with Foundation of the City of Viladecans, extension of the integrated care approaches to other domains, especially in the area of fragile dependant patients with social needs.

Current projects related to Call 1. Obj. 2.2 Foundation City of Viladecans -Telemedicine Interreg. Monitoring at home of frail chronic elderly (chronic disease, dependant people, in demand of social support). ->All stakeholders on board: Hospital, Primary Care centres, Emergency services of Catalonia, social health care centre, social services of the municipality, local NGOs addressing the social needs of the patients, family carers… ->Objectives: Measure the added value of an integrated approach for health care and quality of life of the frail elderly as for: - chronic disease - activities of the daily life - social inclusion Assess the acceptance and readiness of the involved stakeholders

Our objectives Contribute to the deployment at local level in Europe of such ICT integrated social and health services aiming at a better quality of life for frail older chronic patient. As for Spain, seize for instance the opportunity of the current implementation of the “law on dependency” for working on a sustainable business model. Improve our approach by including: ICT for a closer cooperation with family carers. ICT for people with cognitive problems and mild dementia.

In other words Thank you for your attention. We would rather address both of the two goals of the objective 2.2. of the call (chronic disease management… + quality of life elderly people and their families…..)
