What to do with legacy designs? How to address context of a design: People/Procedures/Environment..? How to design for unusual circumstances/unknown events? Is separation/modularity used enough? Does designing to risk address very low likelihood / very high impact events? Are there any tools/techniques which can help to assess whether a design is safe? Can we transfer operational safety culture to design activities? Should we design humans back into the system? Do we feel standards cover safe design sufficiently? How to deal with different sectors maturity in design? What scenarios should be considered for a safe design?
