GAME REPORTS

PURPOSE Provide the primary system for reporting serious penalty infractions Used by: USAH Affiliate Bodies to enforce supplementary discipline and progressive suspensions. USAH Officiating Program representatives to review penalty trends and game management practices Document serious or unusual incidents that occur before, during, or after a game (suspended game, injury, etc.)

WHEN TO FILE Game reports must be written and filed for: All major penalties Game misconducts Match penalties Any serious incident or unusual event (injured player, suspended game, player eligibility question, etc.)

WHO SHOULD FILE Game reports must be filed by: The referee in the Three-Official System The senior referee or referee assessing the penalties in the Four-Official System Only one game report is necessary for each incident Game reports should be filed within 48 hours after the game.

PROCESS Obtain a hard copy of the Scoresheet or use phone to take a picture Use Scoresheet as the reference for filling out the USA Hockey online Incident Report System dialogs Access the USA Hockey online Incident Report System at USAHockey.com (drop-down menu under OFFICIALS tab). Sign-in with your USA Hockey Courses Profile

GET IT RIGHT Fill out all of the data in the online dialogs completely Ensure the narrative portion includes sufficient detail regarding-- Who was involved. Who were the principals? Others involved? What happened. Describe the incident and events leading to it When it happened. At what point in the game? Where it happened. In the play? Behind the play? On the bench? Why it happened. What events led up to the incident? How it happened. Who did what to whom? Stick to the facts -- Unsubstantiated personal opinions, personal attacks, and “moralizing” are not appropriate

GET IT RIGHT Write the narrative portion of reports so that readers who were not present can readily understand what happened Avoid making recommendations or suggesting future courses of action. This is the function of the Affiliate Association(s) A good report provides the Affiliate Associations with all the information needed to make fair and sound decisions Incomplete, inconsistent, vague, or otherwise unsatisfactory reports will be returned for revision

Who Gets the Reports? The USA Hockey Affiliate Association(s) having jurisdiction over the offending team(s) Affiliate Association(s) determine which additional parties will receive copies of game reports. These may include— State and/or local Supervisors of Officials Affiliate staff members League and/or team officials
