Introduction The correct name for this condition is a peri-anal haematoma. It causes severe pain and discomfort and forces the unfortunate victim to seek help urgently. The treatment of choice is incision and drainage of the haematoma. The procedure is simple and can be done in your consulting rooms.

Technique Lithotomy position is recommended but is not essential. A dental syringe and needle to administer a local anaesthetic works best. Inject about 1ml of 1% lignocaine subcutaneously or submucosally into the haematoma. (If injected correctly, a pale area forms). Incise the haematoma and expel the clot by squeezing it. There is usually very little bleeding. Apply a loose gauze swab or a sanitary pad.
