Need for Services Obese children are more likely to become obese adults Obesity complications include: Heart disease Stroke Hypertension Death High Cholesterol Diabetes

Needs Assessment External Environment 13.3% children obese in Michigan between ages 2-5 years Higher prevalence of obesity in Michigan than US 70%-80% obese children become obese adults Big Rapids area has high rates of obesity and inactivity Internal Environment No childhood obesity reduction groups in Big Rapids area Need for obesity reduction services providing dietician, sports training and nursing care Need for collaboration with schools and parents/children

Mission Statement Growing Fit is dedicated to improving the health of children between the ages of 5-14 years in the Big Rapids, Michigan area by providing education and support to families to prevent or reverse childhood obesity

Create an active lifestyle for Organizational Goals Create an active lifestyle for Children! Assist families in becoming motivated and increase knowledge of events in the community Enhance community support programs that will aid families in leading an active lifestyle Ensure children have after school activities to help reduce the risk of childhood obesity Make sure education is provided to parents of children ages 5-14 years in regards to making healthy food choices in turn assisting in a healthy lifestyle

Organizational Objectives To decrease the risk of childhood obesity in children ages 5-14 in the Big Rapids, Michigan area To educate parents and children on making healthy food choices to decrease the risk of becoming obese To collaborate with the community recreation department in creating new high energy after school and summer activities To implement nutritional guidelines into after school / summer activity programs

Organizational Chart Executive Director Registered Nurse Registered Dietician Sports Trainer Director of Nursing We offer services in the least complicated manner possible. Using a phase one shared governance structure lets the nurse executive retain decision making authority while staff is responsible for practice issues and determining roles, functions and processes.

Job Descriptions Registered Nurses Executive Director Sports Trainer Collaborate care between sports trainer and dietitian. Provide professional care to clients using the nursing process to achieve desired outcomes. Report and collaborate with Director of Nursing Provide assessments of children in the program Maintain client plan of care Executive Director Perform leadership functions of governance and decision making. Coordinate all planning, organization and directing of financial funds. Collaborate with all staff member and run day-to- day operations. Maintain a safe and effective environment where obesity prevention is a shared goal. Sports Trainer Provide direct and indirect care to clients Identifies client needs, skills and knowledge needed to increase physical activity Collaborate with the dietician and nursing staff. Promote community events , Director of Nursing Provide direct and indirect care to clients. Program and employee oversight and care plan approval Collaboration with executive director for budget decisions Collaborate care with nurse practitioner, dietician and nursing staff. Registered Dietician Develop specialized diets for at risk children Teach patients and families the importance of nutrition, diet and food selection. Evaluate how patients respond to their specialized diets and make adjustments when necessary Teach class about diet, nutrition and food related to obesity management.
