CPQCC sends the hospital/medical center a: Membership Agreement Contact Form Hospital/medical center returns completed: Membership Agreement Contact Form 4 CPQCC sends the hospital/medical center a New Member Orientation Letter with links to the following: CPQCC Manual of Definitions and Forms CPeTS Manual of Definitions and Forms CCS Supplemental Form and NICU Activity Log HRIF Manual of Definitions and Forms Electronic Data Submission Instructions & Specifications (EDS) 5 CPQCC enters new member’s information into the NICU Database Congratulations! Once you receive the New Member Orientation Letter, you are officially a member of CPQCC! 6 7 8 CPQCC Data Center Team conducts an orientation session with the new member. CPQCC Data Center Team works with the new member to submit and validate first three data entries. New member begins collecting and entering data.
