FSA documents follow-up on the Assistance Referral form and in Child Plus. Parent completes a Parent Referral for Wellness form, FSA sends School Readiness Coach an Internal Communication Memo. School Readiness Coach works with ISD to determine best resource for support. Community Resource Agency Campus wellness support School Readiness Coach follows-up and documents in Blue Folder. School Readiness Coach follows-up with parent/guardian and documents in Child Plus & on Assistance Referral form.
