Screening=Problem ID Diagnostic=Problem Analysis Progress Monitoring=Plan Development & Evaluation Outcome= Plan Evaluation and Accountability [MY NOTES: Activity: Hand out this grid and give participants time to self-assess the system that they have in place at the district and building levels noting weaker areas and adding to Action Plan item list as desired.] Questions to ask: What assessment tools do you have?
